AI helps in accurate diagnosis but have challenges regarding data safety, ethical issues – Syed Adnan Sohail

JMDC/Sohail University Symposium on Artificial Intelligence

KARACHI: Jinnah Medical & Dental College in collaboration with Sohail University organized an international symposium on “Integrating Artificial Intelligence into Basic and Clinical Sciences” on May 24th 2025. Speakers included numerous physicians from Pakistan who have special interest in AI besides a few guest speakers from overseas. Advantages of AI, its benefits in early diagnosis, treatment planning, challenges and opportunities it offers besides issues related to data safety and ethical issues were highlighted by various speakers.

Dr. Asif Principal of Jinnah Medical & Dental College in his introductory remarks highlighted the use of AI in teaching, training and research. It is going to change the future of healthcare and future healthcare professionals will face some challenges. We are promoting the use of AI at undergraduate level. AI, he said, will never replace human mind but it will empower researchers. He advised the participants to be curious and ask questions to the experts and benefit from their presence. AI is a different world; it helps in fast diagnosis and reduced lot of time in scientific writing as well.

It has also brought some responsibilities with it. We are committed to patient welfare hence we have to use AI with responsibility, he added.MS Shehla from Sindh Higher Education Commission also briefly addressed the participants.

First Session

Prof Rehan Ahmad Khan from Riphah international university Islamabad in his keynote address highlighted the Role of Customized Chatbots in surgical decision making. He pointed out that one can make one’s own Chatbot and use it in decision making. Today Chat GPT is more knowledgeable than the physician. Generic Chatbots can be customized. One can select one’s own topic and create a Chatbot. One can conversate with a patient when he or she is referred for surgery. Document it in research. Big data can fix the problem. AI is helpful in making bedside decisions, emergency decisions. It helps in operations and it helps in reasoning. We use AI to validate our decisions. In scientific writing it will only support you but it cannot be listed as author. It is your responsibility to take decision. You save your own material. It is evidence based with seven steps. You can write your own project, ask questions. We are not using it because of explainability and validation. You have to be skeptical while using Chatbots. Studies have shown that ChatGPT matches the physician in decision making. It can be tailored to match needs in surgical decision making, he remarked.

Dr. Fahad Haroon from Karachi X-Rays spoke on the Role of AI in Diagnostics. He discussed AI in machine learning, Deep learning and stated that it analyzes images from X-Ray, CT and MRI. It assists in timely diagnosis. It will never replace the radiologists but it can assist them and help them. They can make diagnosis and then use AI to confirm their diagnosis, he stated.

Dr. Fahad Haroon said that some of the core values like kindness, compassion, forgiveness, self-reflection, continuous learning and professional growth are important. Benefits of AI include early detection, stage-1 cancer small lesions can be picked up which are missed by radiologists and physicians. It is also helpful in Optional radiology dosing, improved patient care and save patients’ lives.

Challenges in AI include differentiation between Benign vs malignant not possible besides infections vs cancerous lesions, Lack of understanding, cognition, emotional intelligence are some of the problems. On can go through routine cases and use it to reduce work load. Studies have showed that about 20% of radiologist are using AI in in their practice. Number of patients is increasing but there is no appropriate increase in the number of radiologists who are overburdened. AI can be used to pick up cancer cases early. Reporting time is reduced by 50% but it has also increased error by 16%.

AI has an important role in Storke i.e., identification of early infarct by AI which is missed by radiologists. It can identify which vessel is being occluded, the thrombus can be removed and patient can be saved. With the help of AI, the patient treatment started within fifteen minutes. He also showed some suspected intracranial hemorrhage slides which can help surgeon save the patient.

Dr. Sohail Zafar from SIUT was the next speaker who made a presentation on Robotic Surgery. He discussed in detail the key capabilities of AI. Learning, Receiving, Perceptions and language understanding. How it works, from Data to Decisions making was also discussed starting with Data input, module, Training, data processing, prediction and action. He then referred to Narrow, General and super intelligent AI. It has proved to be superior than human intelligence on all aspects. It is more complex. Robots are now being used everywhere. AI is being used in business, automobiles, Defense industry. CT integration, Robotic Surgery is a routine. Speaking about the use of AI in Medicine, he said, one can put ECG and it will give you the diagnosis. Robotic assisted surgery is expanding. AI has revolutionized Healthcare. It supports in clinical decision making, risk prediction, medical imaging and diagnostics, Triage, Pre consultation screening. It is being used for asking questions, taking history prior to examination. It can be used to ask relevant questions. But patients fear that when robotic surgery is being performed doctor will not be there. It empowers the surgeons to do surgery quietly. AI is being used in pre operative and post operative scenarios. It will help identify malignancies which are missed by naked eye. He also talked about CT integrated robotic surgery. It is being used in screening and collection of data.

Limitations of AI were also discussed. It included lack of common sense, dependence on data quality. Bias and fairness issues, legal and regulatory uncertainty besides ethical issues. Questions being asked include who is responsible when we use AI.
Dr. Zafar Sayeedi from USA an Alumni of JMDC discussed the Use of Robotics in Oral and Implantology surgery. He shared the salient features of YOMI Robotic Implant Surgery System. It offers guided implant placements.

Discussion

Replying to a question as to how to stabilize the head as movement could be a problem and challenging while doing implants, he said the head is fixed and it rotates with the machine. This system is currently being used by 189 dental surgeons. Dr. Sohail Adnan asked how AI is going to reduce healthcare Cost. The panelists responded by stating that since it will be helpful in early diagnosis, timely treatment can be initiated which will reduce the hospitalization that will reduce the overall healthcare cost. It was further stated that we are mor concerned with quality of care while cost is not the consideration.

Dr. Fahad Haroon remarked that the situation was changing every year. In future all electromedical equipment will have an AI component Cost will reduce eventually. It is not the cost but improvement of care and quality of life is important. Another participant asked if a mistake is made by the robot who will be responsible as Robots can make mistakes. The panelists opined that we need someone to monitor all this and look at all these important issues for which a Regulatory body is needed.

Session-II

Prof. Adnan Kanpur Wala from Karachi Institute of Medical Sciences spoke on “Harnessing AI for Smart Research: From Literature Search to Manuscript Writing”.AI tools, he said, are your colleagues, it will help you assist you. Ask questions quickly. Get synthesis evidence in minutes. The number of research paper being published with the help of AI tools are increasing at a fast pace. Using AI one can save lot of time in scientific writing. According to AMA 66% physicians are using AI in 2024. AI market is expanding rapidly. It offers time saving and reduces burn out among physicians, researchers and has resulted in growth in publications. AI is being used in patients’ management plans and for Psychology sessions.
Dr. Masood Jawaid Director Medical Affairs at PharmEvo Pharmaceuticals made a presentation on “Scientific Research transformation in AI Era.” He was of the view that we need to learn from history, take risks and accept challenges in research. AI is flying machine in modern warfare and we saw it in the recent Indo- Pak conflict where it was AI and strategy that played a decisive role. PMDC curriculum says twenty-five hours should be spent on introduction to computers. After Google Doctor now patients will come prepared after consulting CHAT GPT hence the healthcare professionals need to be well prepared. There are numerous AI tools, Chat GPT is just one of them. AI is not optional. It is a must for medical research, he remarked.

Dr. Noman Islam’s presentation was on “Integrating AI Tools with moral values”. He pointed out that in the days to come almost 50% jobs will be replaced by AI. Hallucinations by AI can provide wrong answers. He also referred to complex moral judgements. Data is AI fuel. AI can also learn unhealthy stereotype answers. Bias is yet another problem. Adverse use of AI and how to handle hallucinations can be a problem.

Dr. Hasring Ibrahim form Malaysia discussed “Research on Neural Network Medical Imaging disease detection”. AI, she said is being used in diagnosing disease by analyzing images of X-ray, CT and MRI. It is also used for treatment planning. She also talked about precision in diagnosis, empowering in work flow. Cost effectiveness, data privacy concerns. How to integrate it with the existing systems is a challenge, she added.

Dr. Shams Nadeem Alam from Baqai Medical University discussed “Future of AI in Academics. Aligning innovations with emotional intelligence.” He pointed out that Emotional intelligence will humanize AI. AI should be compassionate, should be used for human welfare. He also discussed adaptive learning platforms; AI assisted Grading. We as human being, he added, are responsible for it. Learners need connectivity, AI cannot replace empathy, self-awareness, self-regulation, motivation, empathy and social skills. EI enhances engagement, leadership and teaching quality. We need to build Trust and psychological safety. AI helps us move faster and in the right direction. AI supplements learners’ resilience. It will not replace teachers but support them. Educator should provide meaning. Technology is best when it brings people together. Medical Curriculum must include AI, Ethics and Emotional intelligence. Let us promote equity, psychosocial safety, factors responsible for development of AI tools, Emotional literacy and Data with dignity, he added.

Prof. Madiha Sajjad from Islamabad spoke on “Smarter Assessment for Effective learning. How AI is shaping the future of assessment.” In the days to come AI, she stated, is going to take over the job of teachers. Hence what about our student’s future? How do we ensure AI performed assessment and review is valid, ethical and transparent?

How do we teach, guide the students to effectively use AI Tools. She also discussed redesigning the role of teachers using AI in assessment. Lack of transparency in marking could be the cause of confusion. Curriculum outcome is evolving, one size fits all assessment strategies no longer holds. She then discussed the SWOT analysis in detail. Strength can be trained, self-learning, Weakness includes AI literacy issues and Ethical implications. It may also lead to bias perpetuation. Threats include decline in knowledge integrity. Over reliance on automation. Academic dishonesty and functional threats. Opportunities include reduced work load of teachers and students’ empowerment. It can generate MCQs, Scenarios, deliver treatment and provide personalized feedback. In Assessment we must include a reflection section. There will be three roles for the teaches, project designer, feedback curator and ethical moderator. She concluded her presentation by stating that we need to train our faculty in prompt engineering. ethical aspects and Involve students in AI literacy. Future is for AI and human collaboration hence we must encourage and support learners.

Dr. Sobia Ali from Liaquat National Hospital talked about “Evolving role of AI in personalized Education”. She discussed in detail understanding the role of AI in medical education. She opined that we are dealing with Generation Z learners who were born between 1997 to 2012. She also talked about creating interactive case studies. Students’ engagements and class contents can be enhanced. Develop addition of Quiz. We need to redefine the role of educators. These are the challenges and we have to find solution which are available in AI.,
Other speakers who were scheduled to make presentations in the last session included Prof. Shahid Shamim from AKU “Artificial Intelligence: Ethical and Legal Perspectives in Healthcare delivery and education in future” and MS Jane Grant form UK “ AI- the new moral panic”.
