SUKKUR: National Institute of Cardiovascular Diseases Sukkur which is one of the Satellite Centers of NICVD Karachi has so far performed over six hundred open heart surgeries ever since its establishment. Not only that, this purpose built, state of the art tertiary care cardiac facility also performs about six to eight primary angioplasties daily attracting patients from not only all over Sindh but South Punjab and Baluchistan. All this became possible due to the vision and foresight of Prof. Nadeem Qamar Executive Director of National Institute of Cardiovascular Diseases at Karachi and his team. Mr. Khursheed Shah the PPP MNA from Sukkur also played a vital role in the establishment of this center who has also helped established numerous educational institutions in the area.

I was in Sukkur in connection with a meeting organized by Pakistan Cardiac Society Sukkur Chapter in collaboration with Pakistan Aspirin Foundation on September 29th and took some time to visit this state of the art facility. It proved to be a fascinating experience. The center has a busy interventional cardiology programme and most of the patients which are entertained here do require some interventional procedure. It was an initiative of NICVD Karachi that it convinced the Government of Sindh to establish a chain of Satellite Centers and Chest Pain Units all over the province. Hence as a result now NICVD’s network includes ten Cardiac Centers and eighteen Chest Pain Units making it World’s largest Cardiac Healthcare Network. During 2019 NICVD performed 8,956 primary angioplasties at Karachi alone and now over fifty thousand primary angioplasties have been performed at NICVD Karachi and its various Satellite Centers benefitting patients not only from Sindh province but also from Punjab, Baluchistan, Khyber Pukhtunkhawa as well as Azad Kashmir. A vast majority of the patients get absolutely free treatment.

The state of the art cardiac care satellite centers are located at Layari, Larkana, Tando Mohammad Khan, Hyderabad, Sehwan, Sukkur, Nawabshah, Mithi, and Khairpur. During this visit to NICVD Sukkur Dr. Hashim Kalwar was kind enough to take me on a guided tour of this facility visiting the Emergency, Cardiac Cath Lab, Operation Theaters, Medical and Surgical Intensive Care Units, Adult and Paediatric OPD and different wards. Replying to a question Dr. Hashim Kalwar said that there are three consultants one associate and two assistant professors and this center provides round the clock service. We have five adult cardiologists, four consultants are doing their post fellowship while three to four clinical fellows are available most of the time. On the whole twelve to fifteen members faculty is present all the time. There is no waiting time for Echo, ETT and other diagnostic facilities. Before Covid19 pandemic their daily attendance in the Out Patients Department was over eight hundred but now in post Covid era, about three hundred patients visit this facility daily. We have twenty four beds in emergency, six beds for Triage in the emergency, at present over two hundred are functional in this eighty story building. We also have an electrophysiologist in the team. Though we perform six to eight primary angioplasties every day but today he said we had eighteen primary angioplasties of which thirteen were done at night. He further stated that they mostly entertain those patients who require some intervention and since the treatment is absolutely free, patients also come from South Punjab and Baluchistan. Some of them even get their investigations including coronary angiography done from Rahim Yar Khan and then come here for cardiac intervention procedures.

We also had a few Paediatric patients who underwent cardiac surgery here. PDA and ASD are managed here regularly. Teams come from NICVD Karachi regularly to manage the cases. Since our cardiac intervention team is ready round the clock, our door to balloon time is just about twenty minutes. Our surgical mortality is about 1.3% while mortality in intervention cardiac procedures is about 1.1%. Paediatric and adult OPDs are located on first floor. At an average about ten cardiac surgeries are performed weekly. There are twenty four beds in the ICU, we have a Coronary Care Unit, Step down CCU, Post OP ward and all the beds are fully monitored. All ICU and CCU beds has the facility of ventilator as well. The hospital has about thirty ventilators out of which three to four are always in use. Patients visiting various Chest Pain Units in the province are referred here, he added.

Giving details of the facilities available at NICVD Sukkur Dr. Hashim Kalwar mentioned cardiac emergency, cardiac surgery, Primary PCI, Electrophysiology, Angiography, Echocardiography, Angioplasty, cardiology for adults as well as Paediatric patients. Paediatric Cardiac surgery has also started here and all this has come as a great relief to the cardiac patients who require various cardiac interventions.
