There are so many methods available to avoid the pregnancy. The oldest and most primitive methods are the physical ones as coitus interrupts, or withdrawal method. Then comes the local appliances and medicines. The main idea is not to allow any male germ cells i.e. spermatozoa to enter the female womb or uterus. That is done either by ensheathing the penis by a rubber condom commonly known as French Leather or by covering the entrance to the uterus by means of a tampon or a cap commonly known as Dutch cap or a diaphragm or by introducing a sperm killing medicine into the vagina before the sexual intercourse. There are hundreds of such medicines available in the market with the different trade names in form of the vaginal douche, tablets, foam, jelly, cream and some of them contain the mercury salts in them. Lately another method was introduced to discourage the embedding of the ovum in the uterus, if sperm has already entered the womb. That is done by introducing a foreign body in the uterus in form of a plastic coil or a metallic ring so called I.U.D. (intra uterine device or lippes loop). Surprising enough this method has been used for centauries to procure the abortion but nobody thought of it as a mean to check the pregnancy. Then came oral contraceptive which men always dreamed off. Nobody knows how they work but there are just theories. If a woman takes them regularly then there are very little possibilities of pregnancy. Whatever the brand name of the drug may be, they are mixture of estrogens and progesterone hormones in different proportions. Lately the most popular combination used for contraception is ethinylestradiol with levonorgestryl.

Besides these conventional contraceptive methods there are surgical methods as well to stop the birth. Castration by removal of the testis in male or ovaries in female are the oldest methods but they are done for the animals only and have no place for the modern civilized society. The ways used in the human beings are to stop the germ cells to come out in the other world by blocking or disconnecting the channels of transmission which lay between the place of production and site of consumption i.e. the ovarian tubes in females and vasdeferens in males. Except surgical sterilization in males, none of them is absolutely reliable especially in such a fertile couple in whom according to an old English joke, the woman will get pregnant even by just a hand shake with her husband. What a way to adopt? It depends and differs from person to person. A one method suits tom, it might not suit dick or harry. Then other factors like age, excitability, nationality, social customs, morality, and the housing problem also come in the way. It is an admitted fact that in the advancing age like every other thing , the sexual excitability also decreases especially in poor countries like India and Pakistan and more so in poor class where diet is not well balances and contains very little proteins. Then morality and the local customs and housing is such that a woman cannot take active part in exciting the male and the couple cannot have  well-arranged and planned sexual Intercourse. Under such circumstances physical methods, local contraceptives and devices are out of questions which require high excitability, extreme privacy, cooperative wife, disciplined children and the understanding elders in the house.

The contraceptive pills are the best amongst the conventional contraceptives but have failed in this country because our ladies by nature shun taking any medicine regularly. Moreover they are bit expensive as well. About IUD there is lot to be said about, the device which never had been popular in the country of its origin. Surgical sterilization in females is done either by laparoscopic or operative method (per abdomen or paravaginal) for which you need a proper anesthesia, expert hands and hospital beds which are already in shortage for our sick patients. So what should be the better than vasectomy for our country? an operation which is so minor, so easy, but so much effective. You don’t need a highly specialized operation theater. Though so many minor complications are described but in psychologically stable people there are very few of them. This statement I’m making after conducting more than one thousand vasectomies.  

Tail Piece: According to a science fiction “inferno” written by Dan Brown, a new method of contraception in human has been suggested, that is to sterilize all the human being by artificially inducing a pandemic of a vector virus produced by genetic engineering which will attack only human germ cells.
