An Evidence Based Approach to Diabetes
and Endocrine Management

KARACHI: Baqai Institute of Diabetology & Endocrinology (BIDE), recently organized a two-day seminar regarding management of various endocrine diseases. This seminar was part of recently launched online certified endocrine course in collaboration with Cosmopolitan International Diabetes and Endocrinology Centre University of Missouri – Columbia USA.

Dr. Saif-ul- Haque, course director along with Prof. Uzma Khan in his welcome address stated that this course is targeted not only for endocrine fellows and post graduate students but for family physicians as well as those who are interested in dealing with endocrine disorders. He further explained that this seminar will be followed by weekly online lectures as well as interactive case discussions for a period of six months, during which the participants will have the opportunity to interact with eminent faculty members.

Eminent international and national faculty members delivered lectures and conducted workshops during this seminar. Dr. Uzma Khan, a leading endocrinologist at University of Missouri – Columbia USA in her presentation regarding Approach to a patient with thyroid disorder- talked about various presentations of thyroid disorders, diagnostic work up and more importantly about the interpretation of thyroid function tests. Dr. Musarrat Riaz, consultant endocrinologist at BIDE in her presentation about thyroid disorders in Pregnancy discussed latest guidelines regarding management of thyroid disorders during pregnancy and the differences in normal TSH levels during pregnancy. Consequences of undiagnosed and untreated hypothyroidism leading to various maternal and fetal complications were also highlighted.

Dr. Tasnim Ahsan, Prof. Emeritus JPMC discussed in detail various disorders of sexual differentiation. She presented real life cases from her clinical practice and opined that these cases mostly remain undiagnosed and neglected for a long time with serious consequences, thus emphasizing the importance of increasing awareness about this condition. In her concluding remarks she observed that the society deals with them very harshly although it’s not their fault the way they are born, therefore we the health care professionals should treat them with compassion.

Dr. Sobia Sabir from Peshawar talked about Calcium and Vitamin D disorders and various therapeutic options available for management. Prof. Zaman Shaikh in his presentation about Polycystic Ovary Syndrome (PCOS) outlined the historical account as well as different diagnostic criteria’s which has evolved over many years regarding PCOS. He emphasized that PCOS is a clinical diagnosis and other conditions mimicking PCOS should be ruled out like hyperprolactinemia, hypothyroidism, Congenital adrenal hyperplasia, Cushing’s syndrome etc. before making a final diagnosis. Treatment of PCOS depends on the presenting complaints as well as the wishes of the patient, he further opined. Dr. Salman Rasheed from USA discussed various Disorder of thirst and fluid balance including Hyponatremia and Diabetes Insipidus.

Dr. Aneela Darbar, an eminent neuro surgeon from Agha Khan University Hospital talked about Neuroendocrine tumors challenges before and after surgery. She specially focused on prolactinomas and various techniques used for pituitary surgery, However she clearly emphasized that first line management of prolactinomas is medical therapy and surgery is only indicated when medical therapy fails or is not tolerated. Prof. Yakoob Ahmedani talked about insulin initiation and intensification regimen through a step by step approach through different cases. Dr. Zahid Miyan conducted an interactive workshop regarding hypoglycemia which was very well received by the participants as they were made to experience various symptoms of hypoglycemia. A number of cases were also presented by the participants during this seminar which were discussed with senior faculty members including Prof. Naeem-ul -Haq, Prof. Zaman Shaikh and Prof. Uzma Khan.
